SUMMER CAMPS 2011 SPONSORED BY  THE CAGE

SUMMER CAMP PRICES

-  Payment is required by July 1st to hold a spot

-  Each session will be limited to a maximum of 30 campers

-  Current cage members will be offered a 10% discount on all camps

-  All campers will receive a 2011 Cage summer camp T-shirt

-  The fifth day of each week will be held in case of inclement weather

-  If the weather is cooperative we may have a “surprise” in store for the 5^th day

Agreement and Waiver:  I understand that participation in this program constitutes a risk to my child.  I understand the risks associated with this establishment and hereby release The Cage, Dan Sylvia, The Sylvia Companies Inc, RYBL and any of it’s employees or instructors of any and all liabilities that may occur from injuries associated with the risks of participating in this activity however they may happen.  By signing this form this shows that I have read and understand and completely agree with this waiver and with the rules and regulations associated with this establishment.  I further agree that I am participating in this activity at my own risk.
